FACILITY USE FORM

 

This form needs to be completed if you wish to use any of the facilities of the Village of Savona including the ball fields. Please return completed form (original signatures) to the Village Clerk's office with certificate of insurance and payment.  The person who signs the application must also be the insured.

If you want to use the fire department, you first need to contact them directly (President Jody Graham at 583-4346) for availability, fee, and then return the completed application with certificate of insurance to the Village Clerk's office.
